Financial Responsibility Agreement
PAYMENT OF FEES/PROMISE TO PAY
I understand that when registered for any class at 缅北禁地 or receive any service from 缅北禁地, I accept full responsibility to pay all tuition, fees, and other associated costs assessed as a result of my registration and/or receipt of services. I further understand and agree that my acceptance of these terms constitutes a promissory note agreement (i.e. a financial obligation in the form of an educational loan under U.S. Bankruptcy Code §523(a)(8)) in which 缅北禁地 is providing me educational services and deferring some or all of my payment obligation, and I promise to pay for all assessed tuition, fees and other associated costs by the due date.
I understand and agree that if I drop or withdraw from some or all of the classes for which I am registered, or if I am dropped for non-payment or for failing to comply with the Student Conduct Code, I will be responsible for paying all or a portion of the tuition and fees in accordance with the University’s policies on dropping classes or withdrawing from a term (and, if applicable, in accordance with any Continuing Education refund policies). I understand that these policies are incorporated herein by reference. I further understand that my failure to attend class does not absolve me of my financial responsibility as described above.
Third-Party (Sponsor) Payments: I understand and agree that if my third-party payer or sponsor (e.g. an employer, scholarship provider, or other agency) fails to pay or withdraws their promise to pay my charges, I will accept full responsibility to pay all tuition, fees, and other associated costs assessed to my student account as a result of my registration and/or receipt of services.
DELINQUENT ACCOUNT/COLLECTION
Financial Hold: I understand and agree that if I fail to pay my student account or any money due to Cal State East Bay by the scheduled due date, the University will place a financial hold on my student account. I acknowledge that this hold will prevent me from registering for future classes and/or receiving my diploma upon graduation.
Late Payment Charges: I understand and agree that if I fail to pay my student account by the due date, 缅北禁地 may assess late payment fees or charges on my past due balance.
Collection Agency: I understand and accept that if I fail to pay my student account or any monies due, 缅北禁地 may refer my delinquent account to a collection agency. I further understand that if my account is sent to an outside collection agency, I may be responsible for additional collection costs, and my delinquency may be reported to one or more credit bureaus. If a lawsuit or legal action is filed to recover an outstanding amount, I will also be responsible for any legal costs incurred by the University in such efforts (e.g. court fees).
COMMUNICATION
Method of Communication: I understand and agree that 缅北禁地 uses my official university email account (e.g. my Horizon email address) as an official method of communication with me. Therefore, I am responsible for reading the emails I receive from 缅北禁地 in a timely manner and responding or acting as needed.
Contact: I authorize 缅北禁地 and its agents and contractors to contact me at my phone number, email address, and/or mobile device regarding my student account (including delinquent charges or other debt I owe to the University). I authorize the use of automated telephone dialing equipment, artificial or pre-recorded voice or text messages, and personal calls or emails by the University or its agents to contact me in these matters. If I receive such automated calls or texts, I understand that I may revoke my consent for that method of contact at any time by submitting a written request to the 缅北禁地 Student Financial Services Office.
Updating Contact Information: I understand and agree that I am responsible for keeping 缅北禁地’s records up to date with my current physical address, email address, and phone number. I will promptly update my contact information in MyCSUEB (the student online portal) whenever I have changes. Upon leaving 缅北禁地 for any reason (graduation, transfer, withdrawal), it is my responsibility to provide the University with updated contact information for purposes of continued communication regarding any amounts that remain due to 缅北禁地.
FINANCIAL AID
I understand that any financial aid I receive is contingent on my continued enrollment and attendance in each class for which the aid was awarded. Any financial aid award labeled as “estimated” or “anticipated” does not represent a guaranteed payment; rather, it is an estimate of the aid I may receive if I meet all requirements of the aid program. If I drop or withdraw from any class, I understand that my financial aid eligibility may be recalculated and that some or all of my aid may be revoked accordingly. If my aid is reduced or revoked for any reason, I agree to repay any aid that I was not eligible to receive and that was disbursed to me, in accordance with federal, state, and institutional regulations.
I authorize my financial aid (including scholarships, grants, loans, waivers) to be applied toward any and all charges assessed to my student account, including tuition, fees, campus housing, meal plans, health center charges, library or parking fines, and any other institutional charges, in accordance with the terms of the aid programs. I understand that all scholarships, grants, waivers, or third-party payments awarded to me will be credited to my student account and applied toward any outstanding balance I owe. I further understand that my receipt of these funds may be considered a financial resource (per federal Title IV regulations) that could reduce my eligibility for other financial aid. If my eligibility for other aid is reduced as a result of receiving such resources, and if that other aid has already been disbursed to my account, the excess funds may be reversed and returned to the funding source. In that event, I agree that I will be responsible for paying any remaining balance owed to 缅北禁地 after such adjustments.
METHOD OF BILLING
I understand that 缅北禁地 does not mail paper billing statements. I am responsible for viewing my account balance and billing information online through MyCSUEB and for paying my student account on time. I understand that failure to review my account or receive a billing notification does not excuse me from paying my total amount due by the deadline. I will view my account regularly by logging into MyCSUEB (Student Center) and using the Account Inquiry or payment links provided.
BILLING ERRORS
I understand that any administrative, clerical, or technical billing errors or omissions made by 缅北禁地 will not absolve me of the obligation to pay the correct tuition and fees or other financial obligations assessed to my account. I agree to promptly notify the Student Financial Services office if I believe there has been a billing error, and to pay the corrected amount by the due date once notified.
REJECTED PAYMENTS
If a payment I make to my student account is returned or rejected by the bank for any reason – for example, due to insufficient funds or a revoked authorization – I agree to immediately repay the original amount of that payment plus any applicable returned payment fees or penalties. I understand that any returned or rejected payment may result in the cancellation of my class registrations and/or suspension of my ability to register for future classes until the matter is resolved. If I have returned or rejected payments, I may lose the privilege of paying with personal checks or electronic payments, and I may be required to use alternative payment methods (such as cash, cashier’s check, or money order) for future transactions.
FAILED PAYMENT AGREEMENTS
I understand and agree that if I fail to comply with the terms of any payment plan or payment agreement I enter into with 缅北禁地, the University may cancel my current class registrations and/or deny me the ability to register for future classes. I further understand that if I default on a payment plan, 缅北禁地 may restrict me from enrolling in any additional payment plans in subsequent terms, requiring that I pay my balances in full by the standard deadlines.
WITHDRAWAL
If I decide to completely withdraw from a term (drop all of my courses for the term), I will follow the official University withdrawal procedures. I understand that my withdrawal does not absolve me of financial responsibility for any charges incurred, and I will remain bound by the applicable tuition/fee refund policies of the University for that term. Any amounts I still owe after any eligible refund will remain my responsibility to pay.
PRIVACY RIGHTS & RESPONSIBILITIES (FERPA)
I understand that 缅北禁地 is bound by the federal Family Educational Rights and Privacy Act (FERPA). This law prohibits the University from releasing any information from my education record (including details of my student account) to any third party without my written consent. This means 缅北禁地 cannot discuss my account or billing information with parents, guardians, or any other person unless I have given authorization. If I want the University to be able to share information from my student account or education records with a third party (for example, a parent or sponsor), I must provide written consent by following the University’s established FERPA authorization process. I understand that I have the right to revoke such consent at any time, in writing, through the same process I used to grant it.
STUDENT AGE
I understand and agree that if I am younger than 18 years old (the age of majority in California) at the time I enter into this agreement, the educational services provided by 缅北禁地 are nonetheless a necessity for me, and I am legally obligated to pay all amounts due under this agreement. This obligation is pursuant to the “doctrine of necessaries,” which means that minors can be held responsible for the reasonable value of necessary services such as education.
